Utah law requires that a death certificate be filed with the local registrar within five days of the death and before the body is buried, cremated, or shipped out of the area. (Utah Code § 26-2-13.) This is usually done the mortuary or cremation organization, and copies can be ordered for your records at the time of filing.
If you need to order copies of a death certificate later, visit the Utah Department of Health. The DOH website allows you to download a mail-in order form or obtain more info about ordering death certificates online, by phone or in-person.
